Human error can lead to major consequences to human health/life, reputation and business. Safety assessment may be seen by some as an avoidable expense, but it’s considerably cheaper than an accident. Human error is regularly cited as the major cause of accidents, it may be routed in all manner of reasons, it may be latent in poor design or from operators intentionally disregarding procedures for good intentions.

We have extensive experience in supporting the production of Safety Cases at each stage of design.

We have arranged and run numerous HAZOPs and HAZIDs for nuclear installations, rail, ships, military equipment, air traffic control. Our team has the ideal mix of engineers, safety assurance and human factors to ensure assessments are effective and efficient.

Our HF consultants are skilled with Human Error and quantification tools and are experienced enough to know which technique is most appropriate for the particular job.

Our team then offers implementable and affordable methods in reducing human error, typically following the ERICPD hierarchy (Eliminate, Reduce, Isolate, Control, PPE and Discipline). 

Our aim is to find the right balance between making a facility that is licensable vs a facility that is operable.


We can offer consultants that have the following skills:

  • Preparing and running HAZOPs and HAZIDs

  • Selection and using the appropriate Human Error Identification technique, e.g. SHERPA, TRACEr, THEA

  • Selecting and using the appropriate Human Quantification technique, e.g. THERP, HEART

  • Qualitative substantiation following Performance Shaping Factors

  • Identifying human error reduction methods, e.g. independent verification, automation where appropriate
